A: Large organic molecules tend to be insoluble in water (like dissolves like). For this molecule to be soluble, you must try to make it as hydrophilic (i.e. water loving) as possible. This is usually accomplished by using an sodium, or potassium salt, or in other molecules, by acidifying or alkalinizing the drug.
In short, if it wasn’t a salt, it would not be soluble enough in the plasma (which is mostly water) and could not circulate to the target regions.

A: Andy is mostly right. Montelukast also know as Singualir is indicated by the FDA for children as young as 2 years old. This dose is in powder form. Montelukast is an anti Leukotriene more specifically, it is a Leukotriene modifier not an antihistamine. In fact it can be taken with antihistamines.
It is indicated for the prevention and treatment of seasonal allergies and asthma.
For a 5 year old that has congestion, yes It is fine unless he has side effects or is allergic to the medication. Did the doctor prescribe any other medications? I am not a doctor but I would have prescribed a decongestant too.

Generic montelukast tablets or chewable tablets are not yet available. she should have something like an Albuterol inhaler too and a rescue inhaler for emergencies. decadron is a steriod that helps too. cold weather exacerbates asthma as does ragweed that is produced this time of year. even with drugs and inhalers, she’ll have “episodes”. most hold asthma at bay but she can have break through episodes. talk to your doctor about it and see if he can make any suggestions or med changes. kids change so quickly that meds that worked one month can have no effect the next since their body chemstries change so rapidily. the good news is that the chance of her growing out of it are excellent since she’s so young.

A: Montelukast (Brand name Singulair) is used for the treatment of asthma. It works by stabilizing the mast cells which are thought to be the major products in the causing of inflammation and constriction of the airways.
Montelukast is the active ingredient and sodium is the “salt” that it is attached to. The reason for binding it to the sodium is for the purpose of allowing the drug to easily penetrate the cell, thus, producing a better effect.
Most drugs are bound to “salts” such as sodium and potassium. Both these have good penetrative properties.

Q: Faktu & Montelukast?
I was recently prescribed Faktu ointment to control my anal itching. My question is, how should I apply this? It came with a tube (which I am pressuming should be inserted into the anal cavity) which I initially did not use due to my hesitation in having anything inserted into my butt. So… i opted to use it as a cream but when the itching did not stop, I took the risk of finally using the tube applicator however, the itching still did not stop. Am I appying it the right way?
In relation to my first question, I’ve had this anal itching for about a month already and have gone through a fecal test which came out negative. Could this itching be a side effect of my long term use of Montelukast (I was prescribed this tablet to control my asthma)? I’ve been taking Montelukast for about 6-7 months already.

A: FAKTU comes in two forms – ointment and suppository. The ointment as its name implies will be applied just like any other ointment – using an applicator tube. While the suppository form is to be inserted in the rectum – it is usually supplied with a rectal applicator to facilitate insertion.
Faktu has combined effects to arrest rectal bleeding, guard against infection, relieve pain irritation and inflammation and promote wound healing in any case of hemorrhoids.
Though itching is one of the possible side effects of Montelukast (in fact it is a serious side effect), I doubt if it will cause just a local anal itching. But we can give a possibility on that as well.
Try doing sitz bath – place tolerable warm water in a basin and sit for about 15-20 minutes or longer as desired, see if it helps somehow.
If itching persists, be sure it is not some kind of yeast or fungal infection. Try to discuss it with your doctor – you may need some topical cream containing hydrocortisone or the like to relieve your itching. Most importantly, what causes the itching needs to be identified.
